Mid-Semester Feedback Survey: Structured Questionnaire

Through this mid-semester evaluation/feedback, I would like to find out what you think of the class so far and what might be improved over the rest of the semester. This evaluation is completely anonymous.  Although this is completely voluntary, I would greatly appreciate your feedback.

The evaluation is divided into three parts: an open-ended section about the best and worst aspects of the class, a quantitative assessment of the class, and a final section for the purposes of self-reflection.

This should take less than 10 minutes and will provide me with information that will be useful for improving your experience in the course.
