Sample mid-semester surveys

We have designed a few sample surveys that you can download and import into your own Canvas courses.

Note: the downloads are meant to be imported to your course, so they are not readable if you unzip them to your computer.

Importing Sample Canvas surveys into your course site 

  1. After downloading one of the surveys below, in the respective Course's Course Navigation, click Settings.
  2. Click Import Course Content button.
  3. In the drop-down menu for Content Type, select the QTI .zip file option.
  4. Choose File, select the downloaded ZIP file you want to import and click Open.

Start-Stop-Continue Feedback Survey  

Check out the sample start-stop-continue survey questionnaire that has three open-ended questions:  

  • What should I start doing to improve your learning?
  • What is hindering your learning that I should stop doing?  
  • What is helpful to your learning that your instructor should continue doing?

Creating an anonymous feedback survey

If you prefer designing your own survey to gather mid-semester feedback, follow the steps below to create a survey.

  1. Select Quizzes from the course navigation.
  2. Select the + Quiz button.
  3. Under the Details tab, give your survey a name, and provide survey instructions in the text box.
  4. Select Ungraded Survey from the Quiz Type menu.
  5. Select the Keep Submissions Anonymous box.
  6. Click on the Questions tab.
  7. Add the questions to your survey. 
  8. When done, select Save & Publish.
