Sample mid-semester surveys
We have designed a few sample surveys that you can download and import into your own Canvas courses.
Note: the downloads are meant to be imported to your course, so they are not readable if you unzip them to your computer.
Importing Sample Canvas surveys into your course site
- After downloading one of the surveys below, in the respective Course's Course Navigation, click Settings.
- Click Import Course Content button.
- In the drop-down menu for Content Type, select the QTI .zip file option.
- Choose File, select the downloaded ZIP file you want to import and click Open.

Creating an anonymous feedback survey
If you prefer designing your own survey to gather mid-semester feedback, follow the steps below to create a survey.
- Select Quizzes from the course navigation.
- Select the + Quiz button.
- Under the Details tab, give your survey a name, and provide survey instructions in the text box.
- Select Ungraded Survey from the Quiz Type menu.
- Select the Keep Submissions Anonymous box.
- Click on the Questions tab.
- Add the questions to your survey.
- When done, select Save & Publish.
